Immunizations

CALIFORNIA Immunization Requirements for Grades K-12 / 

Effective July 1, 2019

POLIO

4 doses  (3 doses if one given on or after the 4th BD)

DTaP

5 doses  (4 doses if one given on or after the 4th BD)

HEP B

3 doses

MMR

2 doses  (administered on or after the 1st BD)

VARICELLA

2 doses

Tdap

1 dose   (required on or after the 7th BD)

 

ACCEPTABLE Immunization Documents: 

  • Yellow Immunization card
  • MD/Healthcare Provider printout  
  • Copy of Blue CA Immunization Card must be signed and dated
  • CAIR printout.

***School Printouts will NOT be accepted***

MEDICAL EXEMPTION - Immunizations 

California state law requires up-to-date measles, mumps, rubella, HepatitisB, tetanus, diphtheria, pertussis, varicella, and polio immunizations for all students. Exemption for medical reasons must be completed through the California Immunization Registry by the student's medical provider and meet current state guidelines. Failure to meet these requirements will result in the student being excluded from school - please visit CAIR-ME website. 

Students MUST have all immunizations at the time of enrollment. Children behind on their required immunizations may be admitted conditionally if they are not currently due for any doses.
