Proof of ImPACT Baseline Concussion Testing (dated after May 15, 2026)
Where to find it: in the signature portion of your AthleticClearance.com profile on the last page of your Home Campus application. Look for ImPACT Concussion Survey.
Athletes are required to have a tetanus booster within the last ten years preceding participation. Student athletes with authorized medical reasons may be exempt from this requirement.
