Physical Clearance Forms

Physicals for 2025-26 must be dated on or after 5/15/25

  • There are four pages included in the physical form linked below. When creating your profile upload the 4th page titled "MEDICAL ELIGIBILITY FORM"
  • NEW FOR 2025-2026: To be cleared to participate, you will be required to upload proof of completion of IMPACT concussion baseline testing. Instructions are included in the signature portion of your clearance profile.

Clearance Process

  1. Create an account and register your athlete through AthleticClearance.com.
  2. Obtain a sports physical through an MD, DO, NP, or PA. Chiropractors are not an acceptable form of verification and are not on the list provided by CIF to clear athletes for participation.
  3. Proof of medical insurance
    • Athlete’s full name must be on the proof of insurance
    • Need insurance? If you do not have your own medical insurance, it can be obtained through Myers-Stevens.
  4.  Proof of ImPACT Baseline Concussion Testing (dated after May 15, 2025)
    • Where to find it: in the signature portion of your AthleticClearance.com profile on the last page of your Home Campus application. Look for ImPACT Concussion Survey.

  5. Proof of Tetanus Vaccination
    • Athletes are required to have a tetanus booster within the last ten years preceding participation. Student athletes with authorized medical reasons may be exempt from this requirement.
